Communicating with breast cancer survivors and their primary care physicians: Developing a new Survivorship Care Plan

Following treatment, breast cancer patients can feel ‘lost in transition’. To address this, a 3-phase qualitative pilot study was conducted whereby we designed, implemented and evaluated a multi-faceted care plan to address the information needs of breast cancer patients. The perspectives of patients, family physicians, and members of the multidisciplinary oncology team were sought pre-implementation to inform the design of the plan, and post-implementation for evaluation purposes.

The care plan consists of both web-based and paper-based tools, and shows considerable promise in addressing the information needs of breast cancer patients.
